Why do you make me cry?

Looking at this painting, I see a young woman underwater, wrapped in red reefs. Reading the title makes me think: what does crying underwater mean? Don't the tears become irrelevant? You are underwater; no one will see the tears. What does it mean? Why cry underwater in the first place?
